All the bilirubin in the blood, both the fraction the liver has processed and the fraction it has not.
What it measures
Total bilirubin adds direct and indirect bilirubin. A small amount is always present, because you are always recycling red cells. The adult interval on this panel runs up to about 1.0 mg/dL. Jaundice, the yellow color in the whites of the eyes, usually appears when bilirubin is several times higher than that. A mild elevation with no other abnormality is a very common finding.
Why it is on the panel
It is the screening net for liver, bile, and red-cell turnover problems. Most mild isolated elevations turn out to be Gilbert syndrome, a harmless slowdown in processing indirect bilirubin that shows up more when you fast.
How to read it
If total bilirubin is high, look at direct bilirubin and at ALT, AST, and alkaline phosphatase. Enzymes normal, direct bilirubin not the cause: Gilbert syndrome moves to the top of the list, especially if you were fasting for this draw. Enzymes high: the liver tests become the main story.
When it runs higher
Gilbert syndrome, hemolysis, hepatitis, fatty liver with inflammation, alcohol-related liver injury, medicines, and bile duct blockage can all raise the total. The companions on the panel, and whether your eyes are yellow, sort that list. Fasting itself nudges bilirubin up in Gilbert syndrome, so this panel’s fast can reveal it.
When it runs lower
A low total bilirubin is not a clinical problem. It does not indicate a vitamin deficiency or a liver that is “too efficient” in any sense that needs action.
