The calcium circulating in blood. It is kept in a tight range by parathyroid hormone, vitamin D, and the kidneys.
What it measures
Most body calcium is in bone. The blood measurement is the fraction outside bone, and a large share of that is bound to albumin. This panel reports calcium and albumin separately, which matters: a low albumin can make total calcium look low even when the ionized, active calcium is fine. The adult interval is the same for men and women.
Why it is on the panel
Both high and low calcium cause symptoms people often blame on something else — fatigue, thirst, constipation, cramps, tingling. Screening finds the mineral problem underneath. Kidney disease, vitamin D deficiency, and parathyroid disorders are the common medical stories.
How to read it
Read calcium with albumin, phosphorus, vitamin D, and creatinine. If albumin is low, mention that when you review a low calcium. A high calcium with a low phosphorus is the classic parathyroid pattern. A low calcium with a low vitamin D is a frequent, correctable pattern.
When it runs higher
Overactive parathyroid glands are the leading outpatient cause of a persistently high calcium. Extra vitamin D or calcium supplements, some cancers, and sarcoidosis can raise it too. Thirst, frequent urination, constipation, and kidney stones are the symptoms people notice.
When it runs lower
Vitamin D deficiency, hypoparathyroidism, low magnesium, and chronic kidney disease can lower calcium. Tingling around the mouth and muscle cramps are the symptoms of a meaningfully low level. A low albumin is the first thing to exclude.
