Mean platelet volume (MPV)
The average size of your platelets. Younger platelets are usually larger.
What it measures
MPV is to platelets what MCV is to red cells: an average size. When the marrow releases platelets in a hurry, they tend to be bigger, so MPV rises. When production is quiet, the circulating platelets are a more typical, slightly smaller mix.
Why it is on the panel
It adds context to the platelet count. Size plus number is more informative than number alone, especially when the count is at the edge of the interval.
How to read it
A low platelet count with a high MPV suggests the marrow is keeping up by releasing young platelets. A low count with a low or typical MPV suggests production may be lagging. Always pair it with the immature platelet result when you have one.
When it runs higher
Larger platelets show up when consumption is high: immune thrombocytopenia, recovery from a low count, and some inflammatory states. A high MPV with a normal count is usually a minor finding.
When it runs lower
Smaller average platelets can be seen when production is reduced, and in some inherited platelet traits. An isolated mildly low MPV rarely changes care.
