Iron

Iron in the blood and the unsaturated iron-binding capacity describe how iron is being transported, not how much sits in storage. Most circulating iron is bound to a protein called transferrin. UIBC measures the empty seats on that protein. The two numbers together sketch whether iron is scarce, typical, or abundant. They are read with the CBC, because iron deficiency shows up in red cells as small, pale cells and a rising RDW.

What this group shows

  • A low iron plus a high UIBC is the transport pattern of iron deficiency. The blood is short on iron and the carrier protein has room.
  • A high iron plus a low UIBC fits iron overload more than it fits “I ate a steak.”
  • These two tests do not include ferritin, the storage protein. They still catch the patterns that line up with anemia on the CBC.
